Ideally you would need to have stopped growing, otherwise there would be little point or it could go horribly wrong later in life. So it depends on the person, I've seen people have them done as young as 14, but this isn't really recommended. I'd say 18 as a minimum, but it comes down to the ethics of the doctor performing the surgery.
At least 14 with parental consent, but most doctors won't because you're body isn't finished growing. My best friend got hers when she was 15, but as I said the doctor only was okay with it for special circumstances.
18 for sure lol but some girls in my school are like 14-16 and got theirs but they get infections all the time wen they get sick

I think its 16 but I honestly dont know, everything seems to be getting put up to 18 these days doesnt it?

there is no age limit, how ever you have to have concent from a parent/gaurdian if you are under 18. Hope this helps!
